Welcome to our 2026 Annual Conference Nomination Center

We are excited to host the OACFA 2026 Annual Conference and celebrate the dedicated Home Operators and Residential Facilities that strengthen adult care across Ohio.

This year, OACFA will recognize excellence in six award categories. These nominations are an opportunity for Home Operators to honor one another for their diligence, acts of service, leadership, innovation, resilience, and overall facility performance.

If you would like to nominate a Home Operator and their residential facility, please complete the appropriate nomination form below. To help us process entries efficiently, please submit only one nomination per Home Operator for each selected award category.
